The legal basis for the processing of your personal data is the agreement between you and the Controller, hereinafter referred to as the “Agreement,” which is concluded upon your acceptance of the contract sent in electronic form, followed by its signature at the Controller’s office or branch. The processing of your data is necessary for the performance of this Agreement.
  4. Your personal data will be processed solely for purposes related to the conclusion and performance of the Agreement, as well as for the undertaking of any actions required prior to its execution.
  5. The provision of personal data is not mandatory; however, failure to do so will make it impossible to conclude and carry out the Agreement.
  6. Your data will be stored no longer than necessary, i.e., for a period of three (3) years from the termination of the Agreement.
  7. The Controller may disclose your personal data to the following recipients:
    a) The Social Insurance Institution (ZUS)
    b) Banks, Civil Registry Offices, Hospitals, Mortuaries
    c) Entities with which the Controller has concluded data processing agreements.
  8. The Controller does not intend to transfer your personal data to third countries or international organizations.
  9. You have the right to request access to your personal data, rectification, portability, deletion, and the restriction of processing.
  10. If you believe that your personal data is being processed unlawfully, you have the right to lodge a complaint with the supervisory authority – the President of the Personal Data Protection Office (PUODO).
  11. No automated decision-making, including profiling, will be carried out by the Controller based on your personal data.
